Supply Closet Application Guidelines (Effective 2025)

By submitting this application, you acknowledge and agree to the following terms regarding our Supply Closet support:

Limit on Support (ages 10-24 only for pointe shoe support)
- Beginning in 2025, each household can receive Supply Closet assistance no more than two (2) times within any 12-month period,
  OR
- Assistance requests must be at least six (6) months apart from one another.
Application Review
- All applications are subject to approval based on current resources and demonstrated need.
- Submitting this application does not guarantee that assistance will be granted.
Required Documentation
- You may be asked to provide evidence of current need and/or other supporting information as part of the review process.
